Respiratory syncytial virus RSV vaccines were decades in development. RSV causes lower respiratory infections, particularly dangerous for infants and older adults. An early RSV vaccine trial in the 1960s used formalin inactivated virus and caused enhanced disease in vaccinated children, killing two infants. This tragedy set RSV vaccine development back decades. In 2023, the FDA approved the first RSV vaccines. Arexvy by GSK and Abrysvo by Pfizer protect older adults. Abrysvo was also approved for maternal vaccination to protect infants. A long acting monoclonal antibody, nirsevimab, was also approved for infants. The RSV vaccine breakthrough was enabled by the discovery of the prefusion F protein structure by Jason McLellan at the NIH. The structural biology approach also enabled COVID 19 vaccine development.
