The malaria vaccine RTS,S or Mosquirix, developed by GlaxoSmithKline over 30 years, was recommended by the World Health Organization in October 2021. Malaria kills approximately 600,000 people annually, mostly children under 5 in Africa. RTS,S showed modest efficacy against clinical malaria in its Phase 3 trial, lower than most vaccines but significant given the enormous disease burden. The vaccine targets the Plasmodium falciparum parasite before it enters the liver. A second vaccine, R21, developed by Oxford University, showed higher efficacy in clinical trials, particularly in areas with seasonal transmission, and was recommended by WHO in October 2023. R21 is cheaper and easier to manufacture than RTS,S. Together, these vaccines could save tens of thousands of lives annually. RTS,S was the first vaccine recommended against a human parasitic disease.
