The National Academies of Sciences, Engineering, and Medicine published a Framework for Equitable Allocation of Vaccine for the Novel Coronavirus in October 2020. The framework proposed a four phase allocation beginning with high risk health workers, first responders, and people at highest risk of severe disease, and ending with the general population. The ACIP adopted its own phased approach. States were responsible for implementation, leading to inconsistent rollout. Some states prioritized essential workers differently. The NASEM framework emphasized equity but implementation fell short. Early vaccination data showed disparities by race and ethnicity. The CDC addressed these through targeted outreach and mobile vaccination clinics. By mid 2021, vaccination rates among Black and Hispanic Americans improved. The equity lessons from COVID vaccination have been applied to other vaccine programs including mpox vaccination.
