Thermostable vaccines that do not require cold chain could revolutionize vaccination in developing countries. MenAfriVac was the first vaccine prequalified by WHO for use at controlled temperature chain CTC, allowing storage at up to 40 degrees Celsius for 4 days. Thermostable vaccine technologies include sugar glass formulations, spray drying, and protein engineering. Silk fibroin stabilization developed at Tufts University shows promise. The Gates Foundation has invested in thermostable vaccine technology. Thermostable vaccines would reduce waste, lower logistics costs, and expand access to remote areas. However, developing thermostable formulations is technically challenging and must be done individually for each vaccine. The lack of commercial incentives for optimizing vaccines for developing country conditions has slowed progress. Thermostable COVID vaccine formulations could have dramatically simplified global distribution.
