Measles outbreaks in emergency departments have exposed hundreds of patients. Pertussis outbreaks among healthcare workers have required prophylactic antibiotics and furloughs. Varicella exposures in neonatal units pose serious risks to infants. Hospitals increasingly require healthcare worker vaccination against influenza, hepatitis B, MMR, varicella, and Tdap. Mandatory influenza vaccination of healthcare workers was implemented by many hospitals after the H1N1 pandemic. Some healthcare workers object to mandates. During COVID, healthcare worker vaccine mandates were challenged in court but largely upheld. Nosocomial outbreaks demonstrate that healthcare facilities can amplify disease transmission. The Association for Professionals in Infection Control and Epidemiology APIC recommends vaccination as a core infection prevention measure. Healthcare worker vaccination protects both patients and staff.
