India has operated an Adverse Event Following Immunization AEFI surveillance system for decades. The system was strengthened after introduction of new vaccines under the Universal Immunization Programme. Earlier, reports of infant deaths following pentavalent vaccination in Sri Lanka, Bhutan, and Vietnam had caused concern. Investigations found no causal link in most cases. India faced similar concerns during pentavalent vaccine introduction. The National AEFI Committee investigated cases and published findings. The AEFI system identified rare adverse events including anaphylaxis and syncope. The AEFI surveillance system has been important for maintaining public confidence as India introduces new vaccines. India has one of the largest immunization programs in the world, targeting roughly 26 million newborns annually. Maintaining AEFI surveillance at this scale is challenging.
